Introduction to the Problem

What happens when we’re crowded, and what works to fix it? The speaker will discuss the following points:

  • Discuss the literature that supports boarding of admitted patients as a driving force for ED crowding.
  • Discuss consequences, including impact on morbidity and mortality. Discuss the need to separate ED flow improvement strategies (which do not impact on boarding) from hospital capacity strategies.

Smoothing of Elective Admissions

What drives capacity problems?  The speaker will discuss the following points:

  • Discuss how scheduling of elective admissions impacts on capacity, quality, safety and cost of care as well as contributes to boarding.
  • Introduce the concept of smoothing, how it is applied to elective admissions.
  • Describe institutional obstacles to smoothing and how they can be approached.
  • Provide case examples and demonstrate how this intervention improves access to care, its quality and bottom line.

Enhancing Weekend Discharges

How does discharge activity on the weekend impact on capacity? The speaker will discuss the following points:

  • Describe how enhancing weekend discharges impacts on capacity, flow, ED boarding, and the institutions’ financial bottom line.
  • Describe institutional obstacles and how to address them.

How does inpatient discharge time impact on capacity? The speaker will discuss the following points:

  • Create a sense of urgency within the institution for early discharge.
  • Develop a strategy for success (and maintaining success!) for early discharge.
  • Analyze how systems changes may broadly impact early discharge.

Table Design Exercise

Each table developing strategies to engage leaders across their health care organizations in improving and optimizing hospital flow. Teams will use design canvases to co-create strategies and innovative ideas in a systematic way to produce real results.
