Brief narrative description of case
A 17-year-old male presents to a rural ED via EMS after passing out while on a summer backpacking trip with family. He has an undiagnosed history of HCM and is severely tachycardic and hypotensive upon arrival. Bedside US demonstrates septal thickening with SAM and aliasing, concerning for severe left ventricular outflow tract (LVOT) obstruction in the setting of dehydration.
